How do I make a claim?

Answer:

Once you have received your dental treatment and paid your dentist you'll need to complete a Tesco Dental Insurance claim form. You'll also need an itemised receipt from your dentist and ensure you have proof of payment to enable us to process your claim quickly.

If you would like any help in obtaining or completing a claim form please call us on 0800 678 1794 (TypeTalk 18001 0800 678 1794).

Then return your completed claim form to us (Tesco Dental Insurance, AXA PPP healthcare, Phillips House, Crescent Road, Tunbridge Wells, TN1 2PL) with the itemised receipt from your dentist and your proof of payment. If there are no queries, we will send you a cheque within 10 working days.
